Can I do a name change?
Yes but we will only do it if it is a legal name change ie: for marriage, adoption, or by court order. You will also be responsible for any fees for a duplicate Certification of Completion. Send a letter or e-mail request with documentation of your Certificate of Completion, and marriage license, or adoption order or court order to change your name. We will accept photocopies of these records.
